• Must meet ACA eligibility requirements

  
  

• All spouse and dependent info must be listed according to the IRS Name, birthdate, and SSN must be a match

  
  

• Must have accurate household income according to the IRS

  
  

• Must have a valid residential address

  
  

• Must have a valid signature (scribble is not acceptable)

  
  

• Must have a valid phone number & email address

  
  

• Must NOT have insurance through an employer

  
  

• Customer must be aware that you are enrolling them in healthcare
